Program Overview

OTOKAI is a dementia prevention program built around weekly AI-assisted music composition lessons.

OTOKAI is an innovative cognitive wellness program that uses music to support dementia prevention. In weekly small-group classes, participants use a dedicated app to create their own musical works. Anyone can enjoy a simple music composition experience, and completed works are shared and discussed with fellow participants. Instructors draw upon their musical expertise to provide commentary and feedback on participants’ works.
Through this composition experience, participants stimulate their brains and support the maintenance of cognitive function. The program also promotes social interaction, helping participants discover new interests and build meaningful connections with others.

Small-Group Lessons with Personalized Guidance

OTOKAI lessons are conducted once a week using a dedicated app in groups of approximately ten participants. The small-group format allows instructors to provide individualized attention and support. Online participation is also available, enabling people from across Japan to take part.

Typical Lesson Flow

Step 1: Theme Presentation
A theme is presented through the app. Themes may include photographs, artwork, videos, haiku, poems, quotations, or other visual and written materials.

Step 2: Select AI-Generated Music
Participants enter words inspired by the theme and select the AI-generated musical piece that best matches their interpretation.

Step 3: Presentation and Discussion
Participants present their work, discuss their creative choices, and receive feedback from the instructor.

Step 4: Cognitive Assessment
Cognitive assessments are conducted before the program and again after six months to evaluate whether cognitive abilities have been maintained or improved.

* Because ongoing participation is required, cognitive assessments are not included in one-time events.

The Effectiveness of OTOKAI Has Been Scientifically Validated

A joint study with the Advanced Institute of Industrial Technology in Tokyo demonstrated improvements in frontal lobe function among participants who continued the OTOKAI program. Participants can join with confidence.
The program has been selected for Tokyo’s Strategic Innovation Promotion Program, and a second study is currently underway.

What Is an OTOKAI Instructor?

A dementia prevention professional who applies expertise in music education and music therapy to support an aging society.

OTOKAI instructors facilitate lessons, encourage participation, provide musical commentary and interpretation, and help bring out participants’ creativity while fostering interaction. They communicate musical concepts in an accessible way and support the creative experience regardless of participants’ musical background or level of interest in music. It is a socially meaningful role that helps bring joy, engagement, and well-being to participants’ daily lives.
